Can't find your debit or credit card anywhere? Are there purchases on your account that you haven't made and you are worried your card information has been stolen? Or maybe your card is cracked and just doesn't want to slide through the card reader anymore? Step 1. Contact us immediately here at BRCB (715-284-9448) so that we can temporarily disable your card.
Step 2. Open by clicking on this Lost/Stolen/Damaged Card PDF Form right from your own computer and either print it and fill it out by hand or complete it electronically and then print.
Step 3. Be sure to leave your best contact number so that we can contact you to reissue your new card! Also remember that you will need to SIGN and date the form.
Step 4. Because we know that you are busy, we have multiple options for returning your form. You can simply....
- Bring it to BRCB (drop it into the night deposit if we aren't open);
